Destroy stock with a witness

In short

Expired, damaged or recalled stock is destroyed with a witness and recorded: list the batches, say how they're destroyed, then you and a witness sign with your PINs.

Who: managers and the owner. Controlled drugs: only staff allowed to handle them, and the regulator's officer is named.

Record a destruction

1. From Expiring Stock choose Start next to the batch (or open Destroy Stock with a Witness).
2. Under Items to Destroy, check each batch and Why (Expired, Damaged, Recalled, Temperature excursion, Other). Add Another Batch for more.
3. Choose the method: Licensed waste contractor, Denaturing kit on site or Handed to NAFDAC, and fill in the contractor (or NAFDAC office). The certificate number can be added later; keep the certificate, since it's your proof the stock was destroyed properly.
4. For a controlled drug, add the Regulator officer and their ID number.
5. Choose Sign and Record Destruction, type your PIN, then Continue to Witness.
6. The witness chooses their name, types their PIN, and chooses Sign and Record.

The stock leaves the system when the witness signs. The record and signatures go in the inspection file and can't be edited. If the certificate comes later, open the record and choose Attach Certificate.

If something goes wrong

• "Name the waste contractor."
• "A controlled drug needs the regulator officer’s name and ID number."
• "The witness must be a different person from the recorder."
